PLAN CONFIRMATION: I understand that both Elite Complete and Premier Complete plans include catastrophic protection for major medical expenses. Employees will have coverage for serious illnesses, cancer treatment, major surgeries, and catastrophic events.

How employees enroll

Employee enters Enrollment Code (required) + Last Name + DOB.
System matches and prefills name, DOB, job class, worksite/location, employer contribution.
Employee confirms details → chooses doctor & plan → adds spouse/dependents → e-signs acknowledgments.

Common questions

Data security: CSV upload is via portal (no email). PII is encrypted at rest/in transit.
Mismatch handling: If code+name+DOB don't match, employee gets a friendly "Can't find you" prompt with support handoff.
Dependents: Added by employee during onboarding; contributions apply per your Step 6 settings.
Timing: We recommend 5–10 business days from roster upload to go-live communications.
