1. ACCEPTANCE OF TERMS
By using CareGoRx services, you agree to be bound by these Terms of Service. Your continued use constitutes acceptance of all terms herein.
2. TELEHEALTH SERVICES
Telehealth services are subject to technical limitations and system failures. CareGoRx disclaims all liability for technology failures, connectivity issues, or service interruptions. You assume all risks associated with telehealth technology.
3. MEDICAL TREATMENT DISCLAIMERS
CareGoRx services are not a substitute for emergency medical care. You are responsible for seeking appropriate emergency care when needed. CareGoRx disclaims all liability for treatment outcomes, medical decisions, or health consequences.
4. PRIVACY AND DATA SHARING
You consent to the collection, use, and sharing of your personal health information with insurance carriers, authorized partners, and third-party service providers. CareGoRx disclaims all liability for data breaches or misuse of your information by third parties.
5. APPOINTMENT POLICIES
You must cancel appointments at least 12 hours in advance. No-shows and late cancellations may result in fees and service restrictions. CareGoRx reserves the right to terminate services for policy violations.
6. INSURANCE AND PAYMENT
You are solely responsible for verifying insurance coverage, copayments, and coverage limitations. CareGoRx disclaims all liability for insurance denials, coverage gaps, or payment issues. You assume all financial responsibility for services rendered.
7. LIMITATION OF LIABILITY
TO THE MAXIMUM EXTENT PERMITTED BY LAW, CAREGORX SHALL NOT BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF SERVICES.
8. INDEMNIFICATION
You agree to indemnify, defend, and hold harmless CareGoRx, its officers, directors, employees, and agents from any claims, damages, losses, or expenses arising from your use of services or violation of these terms.
9. DISPUTE RESOLUTION
Any disputes shall be resolved through binding arbitration. You waive the right to a jury trial and class action participation.
10. ASSUMPTION OF RISK
You acknowledge and assume all risks associated with healthcare services, including medical complications, treatment outcomes, technology failures, and service limitations. You release CareGoRx from all liability for any consequences of your healthcare decisions.
11. POST-ENROLLMENT VERIFICATION
For security and fraud prevention, CareGoRx may require post-payment verification calls to confirm your identity and enrollment information. You consent to receive such calls within 24-48 hours of payment. Failure to complete verification may result in delayed or suspended service activation.
12. FORCE MAJEURE
CareGoRx shall not be liable for any failure to perform due to circumstances beyond its control, including natural disasters, government actions, pandemics, or other force majeure events.
